Patient Expectations
We expect our patients to be considerate of our medical providers, staff and other patients.
Patients should make an effort to provide accurate health care history so that we may provide the best possible care tailored to your need.
We expect patients to fulfill all financial obligations for the healthcare that they receive at our clinic.
Patient Privacy
Patient Privacy is of utmost importance to us. We also ask that patients respect the privacy of other patients at our office. Off-hours and off-site appointments are available for additional privacy.
Appointment Necessities
- All Insurance/Worker’s Compensation Information
- Insurance Authorization (if you have HMO insurance)
- All relevant progress notes from your referring health care provider
- Any imaging studies (MRIs, x-rays, etc.) labs and other studies (EMG, etc.)
- Any copay in cash or check
- We do not accept debit cards
Insurance Information
We accept Medicare, Worker’s Compensation, QME patients, and most insurance plans. Currently we are contracted with the following insurances:
- Aetna
- Blue Cross
- Blue Shield
- Cigna
- Medicare
- Coventry / 1st Health
- Health Net
- United Health Care
- Genex – Contracting application is in process
- Tristar – Contracting application is in process
Any person may see Dr. Singh on an individually paid basis and recieve an appropriate form to submit to his/her insurance plan for reimbursement for covered benefits.
We DO NOT accept HMO insurance or Medi-Cal.
